Alcoholism isn't an overnight process. It builds through a repeating cycle that affects the brain in distinct ways at each stage.
Knowing the stages helps you understand how it works and why you can't just stop it at your own will, without support.
How Does Alcohol Addiction Develop?
According to the NIAAA, alcohol addiction can be framed as a repeating cycle with three stages. Each stage is linked to and feeds on the others. This can take place over days, weeks, or months. A person can even cycle in and out of it multiple times in a day, or jump in at any one of these three stages, not always necessarily at the start.
These three stages largely pertain to three areas: reward-seeking system, negative emotional states, and executive function. These are reflected by three main areas of the brain: the basal ganglia, extended amygdala, and the prefrontal cortex.
Stage 1: Experimentation
Most people who develop Alcohol Use Disorder (AUD) did not start out with a problem. Drinking begins as something social, occasional, or curiosity-driven.
Alcohol is used in low-risk social situations, like parties or celebrations.
The person feels relaxed, less anxious, or more at ease around others.
There are no consequences at this stage, and use feels entirely manageable.
For most people, drinking stays here. For others, it does not.
The basal ganglia play an important role in motivation as well as in the formation of habits and other routine behaviors. This is why what starts as a choice begins to feel automatic.
Stage 2: Regular Drinking
Drinking becomes more frequent and starts to serve a purpose beyond socializing. This is where patterns begin to shift.
You use alcohol to relieve yourself of emotional burdens after work.
You build up a tolerance where you find you need to drink more to achieve the desired effect.
Drinking occasions become regular and harder to resist.
You may not recognize that it's an issue, and your friends and family might also fail to notice.
With constant stimulation of your brain's 'reward system', you are programming yourself to want more, more, and more. Soon, innocent cues—like the sound of the clinking of glasses or specific people and places—can trigger a craving by themselves.
Stage 3: Increased Dependence
This is where drinking is more than just a habit, and the mind and body become dependent. Stopping drinking now begins to feel harder.
Cutting back feels uncomfortable or fails entirely despite genuine attempts.
Withdrawal symptoms appear when drinking stops, including shaking, sweating, trouble sleeping, and anxiety.
The person drinks not to feel good but to avoid feeling bad.
Relationships, work, and health begin to show strain.
This is also when hyperkatifeia becomes relevant. This term describes the overactive negative emotional state associated with alcohol withdrawal. It is thought to drive continued drinking as the person seeks relief from that emotional state through alcohol.
Stage 4: Addiction
What was once a choice has become a compulsion. This is the moderate to severe end of the AUD spectrum.
Alcohol dominates daily decisions and priorities.
The person continues to drink despite knowing the harm it is causing.
Cravings are intense and hard to override.
Attempts to stop without help are often unsuccessful.
The prefrontal cortex, the area of the brain responsible for organizing thoughts, prioritizing tasks, managing time, and making decisions, is compromised in people experiencing alcohol addiction. This is why willpower alone rarely works at this stage.
Why This Cycle Matters
Knowing about these stage cycles of alcoholism confirms that it is a disorder that alters the brain: each stage can lead to physical and psychological adaptations in the brain that then increase the probability of the next cycle beginning.
Young people face a particular risk. Research shows that people who start drinking before the age of 15 are at a higher risk for developing AUD later in life compared to those who wait until age 21 or older.
Alcohol during adolescence can interfere with normal brain development, since the brain continues developing well into a person's mid-twenties.
